Client Rights & Consumer Information
In accordance with Texas House Bill 4224 (Section 181.105, Health and Safety Code), Stanford Couples Counseling provides the following information regarding your healthcare records and consumer rights:
1. How to Request Your Healthcare Records
You have the right to access your health records. To request a copy of your records:
- Submit a Written Request: Please send a signed and dated “Authorization to Release Records” form to our office via 4311 Oak Lawn Ave, Suite 350, Dallas, TX 75219 or info@scctherapy.com.
- Format: You may request your records in either electronic or paper format.
- Timeline: We will process your request within 15 business days. Note: A reasonable, cost-based fee may apply for copies and postage.

If you have questions regarding our professional licensing or state regulations, you may contact the Council directly:
